PAYMENT/CANCELLATIONS: During this process you may pay online or in-office. Our system will require you to put a credit card on file in order to reserve your appointment, we ask to please read the cancellation policy, if you are a "No Show" then this card will be used for the cancellation fee.
If you cannot make a scheduled appointment, please contact us.
INSURANCE: If you have health insurance please contact us prior to booking your appointment, so that we may verify benefits and schedule you.
